LPA and AMD in Singapore
A Lasting Power of Attorney (LPA) and an Advance Medical Directive (AMD) are both legal documents for medical-related decision-making.
These documents come into effect should you lose the ability to make your own decisions, and can help ensure your wishes are respected in such situations.
LPAs and AMDs are often confused because both are prepared with the help of a doctor, but they cover very different situations. Below, we explain what each one does, how they differ, and how you can get them certified at any Frontier Healthcare clinic.

What is an LPA?
An LPA lets you appoint one or more people (Donees) to make decisions on your
behalf if you lose mental capacity. It gives you peace of mind that your affairs
will be managed according to your wishes, even if you’re no longer able to decide
for yourself.
What is an AMD?
An Advance Medical Directive (AMD) is a legal document that tells your doctors, in advance, that you do not want extraordinary life-sustaining treatment if you become terminally ill and unconscious or unable to make decisions. It’s made with a doctor’s guidance and can be revoked at any time.
What's the main difference between an LPA and an AMD?
An LPA appoints someone to make decisions on your behalf across healthcare,
welfare, or financial matters if you lose mental capacity for any reason. An
AMD only applies if you’re terminally ill and unconscious, and states that you do not want life-sustaining treatment (it does not appoint anyone to decide on your behalf).
How do I make my LPA?
Log onto the Office of the Public Guardian Online (OPGO) via Singpass to draft your LPA, then visit a Certificate Issuer who will explain the document and certify it. Once submitted online, you’ll be notified when your LPA is accepted and registered.
Do I need a lawyer to make an LPA?
Not for a standard LPA (Form 1), which can be certified by an accredited doctor. A lawyer is only required for LPA Form 2, which involves customised powers.
Who can I appoint as my Donee?
Anyone you trust who is at least 21 years old. You can appoint up to two Donees plus one replacement Donee under LPA Form 1.
Can I change my mind after making an LPA or AMD?
Yes. Both can be revoked at any time while you still have mental capacity, by following the relevant revocation process with OPG or the AMD Registrar.
What happens if I don't have an LPA?
If you lose mental capacity without an LPA in place, your family would need to apply to the Family Justice Courts for a Deputyship Order, which is a longer and more costly court process, before anyone can legally manage your affairs.
What do I need to bring for my LPA appointment?
Bring your NRIC and the mobile phone with your Singpass app installed. The LPA has to be signed digitally via the Singpass app (unless you do not have Singpass). You should also have already drafted your LPA on OPGO beforehand.
Does my donee need to come for my LPA appointment?
No — your Donee doesn’t need to be present. They are required to separately accept their appointment as Donee online via Singpass, before your certification visit.
When does an AMD take effect?
The AMD takes effect only when two doctors certify that you are terminally ill and unable to regain consciousness or communicate.
Do I need an appointment, or can I walk in?
Booking an appointment is recommended to avoid long waiting times.
How long does the certification appointment take?
An appointment for an LPA takes about 15-20 minutes. An appointment for both an LPA and AMD can take 15-30 minutes.
