Dr Chong Chin Kwang

Dr Chong is Director (Healthcare Services) and oversees the clinical operations of the Group’s medical clinic entities. Apart from his Board and corporate management responsibilities, he is also a practising Family Physician.

Dr Chong obtained his MBBS from the National University of Singapore (NUS) in 1994 and subsequently obtained the Graduate Diploma in Family Medicine. He has also attended the SPRING-NUS Advanced Management Programme in 2011. He is a part-time clinical tutor in the undergraduate Family Medicine programme of the NUS Yong Loo School of Medicine (NUS). He has previously served in MOH workgroups – the Community Health Centre (CHC) workgroup and the National GP-IT workgroup.

In addition, Dr Chong was instrumental in pioneering and operationalising the Primary Care Network (PCN) in Singapore, from its pilot phase in 2012 to the current MOH endorsed framework. He is currently the Clinical Lead of Frontier PCN, a network of GP clinics formed from Frontier’s clinics and independent small GP clinics in the community. The PCN model aims to to support private GPs to deliver quality team-based care in chronic disease management.

Dr Lee Wen Yan

Dr Lee is one of the co-founders of Frontier Healthcare Group. He graduated from National University of Singapore (NUS) in 1992 and completed his post-graduate diploma in Family Medicine in 2005.

He started working in the first GP clinic of the Group in 1997 and is still currently practising at the Woodlands branch as a Family Physician. He was part of the management team until 2010 when he took a sabbatical to pursue other areas of interest. It was while working with a Medtech incubator/venture capital firm that spurred his conviction that primary care could greatly benefit from the application of technology to the practice.

In 2019, he joined Frontier Primary Care Network as the Administrative Lead and took on the role of Director (Clinical Planning and Health Innovation) in 2020. He also rejoined as Board member in 2020.

Dr Tham Tat Yean

Dr Tham is currently Board Chairman and CEO of the Group. He is also a practising Family Physician.

He is an Associate Programme Director in the NUHS Family Medicine Residency Programme and a Visiting Family Physician to the NUHS Department of Family Medicine.

He currently serves as part-time Senior Consultant with the MOH Primary and Community Care Division. He chairs the Primary Care Network Council and co-chairs the National GP Advisory Panel. He is also serving in several other Ministry of Health and public healthcare institution committees. He has previously served in the Councils of the College of Family Physicians Singapore (2005 to 2015) and the Singapore Medical Association (2000 to 2006), and was a Board Member of Agency of Integrated Care from 2009 to 2012.

Dr Tham’s current area of interest is in the development of collaborative models of primary care with various public healthcare institutions and community entities, with a vision of transforming primary care to meet the healthcare challenges of an ageing population.
