Yishun Health (YH), Blossom Seeds and Frontier Medical Associates (Canberra) have come together to support residents in connecting and accessing resources in Canberra so that they may live and age well in the community.
To participate, residents simply indicate their interest through a coded poster at the clinic following which RT will get in touch with the residents. Based on the residents’ health goals, connections are then made such as the various activities available atBlossom Seeds’ Active Ageing Centre whom RT works closely with.
Frontier Healthcare is proud to be part of this very meaningful initiative.
