Our PCN provides the following services to support patients on follow up with our clinics:

  • Nurse counselling
    • One-to-one counselling sessions with our nurses can help you to understand your chronic medical conditions better
    • Discuss customized adjustments to your diet and lifestyle
    • Revise various techniques like inhaler use, blood sugar monitoring and insulin injection with our nurses
  • Screening for complications of diabetes
    • Eye screening
      • High sugar levels can damage the small blood vessels in the retina
      • Yearly screening of the eye can pick up complications in the retina before symptoms like blurred vision or loss of vision arise
    • Foot screening
      • High sugar levels can damage the nerve endings and small blood vessels in the feet
      • Yearly examination of the feet can pick up these complications before they lead to further problems like diabetic foot ulcers or gangrene
  • Spirometry testing
    • Spirometry is a lung function test for the diagnosis and monitoring of lung conditions like asthma and chronic obstructive pulmonary disease (COPD)
    • Consult your family doctor if you have symptoms of breathlessness, wheezing or long-standing cough to find out if you would benefit from spirometry testing
